REQUESTING UPGRADE TO WOWZA MEDIA SERVER 2:
PERPETUAL EDITON SUBSCRIPTION EDITION
Thank you for requesting an upgrade of your current Wowza Pro Software license to a Wowza Media Server 2 Software license. We are willing to provide this exchange according to the terms of this Form and your payment of any additional license fee, if required. Please email or fax the completed Form to us as indicated above and retain a copy for your records. Once we approve your request, we will either issue you a new license key(s) or issue you a coupon code for obtaining a new license key(s) from our ecommerce provider. Please be advised that you must delete all copies of the Wowza Pro Software and corresponding license key(s) from your server, computers and other media on which they are installed, stored or archived. You must take whatever measures are necessary to destroy all copies of the Wowza Pro Software and corresponding license key(s), and dispose of the media in an appropriate manner. You must not sell, lease, license, sublicense, rent, assign, distribute, give away, donate or otherwise transfer the Wowza Pro Software or corresponding license key(s), or copies thereof, to any other person or entity. You must cease providing services to third parties using the Wowza Pro Software. If requested by us, you will provide us with written certification that you have completed all required actions. Your license key(s) to the Wowza Pro Software will be considered as “destroyed” and your license to use the Wowza Pro Software under the Wowza Media Server End User License Agreement will immediately terminate when we (i) receive this completed Form from you, and (ii) issue you a new license key(s) or a coupon code for the Wowza Media Server 2 Software.
